Support Worker - Community Houses (Western Bay)

Hours: 35 per week

Salary: £18,000 p/a

Location: Swansea and Neath Port Talbot

The project supports clients in the community to become independent and sustain their accommodation in the future by providing support in in one of our shared housing projects. A key focus of the project is supporting people at imminent risk of losing their accommodation to take the necessary steps to prevent homelessness.

The post holder will provide an excellent and holistic housing support service in line with the expectations of Caer Las, Supporting People and local authority commissioners. This includes being responsible for all aspects of managing a client caseload such as; completing and developing risk assessments, ensuring an outcomes focus, recording progress and next steps, monitoring and measuring client success. This will all be undertaken with a client centred and solution focussed approach which places the client at the centre of the service and enables them to sustain accommodation and move on with their life in a positive way.  

There is a particular emphasis on developing community links with agencies and services that can be accessed and sustained in the longer term once support ceases and this will be an ongoing requirement of the role.

It is essential to have a good understanding of Microsoft Office (Word and Excel).
